Tattoo and Piercing Minor Consent Forms MUST be Notarized

In Florida, a person under the age of 18 must have the State of Florida Department of Health Minor Consent Form signed by a parent or legal guardian and notarize before they are able to get a tattoo or piercing done.  If you get your consent form notarized prior going to the shop it can save you time, money, and frustrating.  When we get called from the shop, we may not be available or nearby so you might have to wait for us AND the shop might have to take another customer while you wait for us so it might take even longer.  If possible, call us prior to going to the shop. We can arrange an appointment to meet at the shop when you are going to have the piercing or tattoo done and get your form notarize.
